ABSTRACT

Objective:To compare the difference of weaning time, comfort and satisfaction during the time, and arterial blood gas analysis in early active exercises of mechanical ventilation patients with myasthenia gravis (MG).Methods:Fifty-six mechanical ventilation patients with MG hospitalized in the Department of Neurology, Tangdu Hospital, Air Force Military Medical University were enrolled from January 2014 to January 2018. All the patients were divided into observation group ( n=30) and control group ( n=30) according to the random number table. Observation group performed active exercise, that encourage patients get out of bed as early as possible. Control group implement passive exercise. Then, analyzed the difference of weaning time, comfort and satisfaction during the time, and arterial blood gas results between this two groups. Results:The weaning time of observation group was (6.2±2.3) days, while (9.8±3.9) days of control group, the difference was significantly ( t value was 5.559, P<0.01). The comfort of observation group were (81.20±4.93) points, while (64.54±2.62) points of control group, the difference was significantly ( t value was15.798, P<0.01). In the observation group, 3 cases were satisfied with the nursing service, 27 cases were satisfied, and the patient satisfaction score was (93.67 ± 2.20) points; in the control group, 2 cases were generally satisfied, 15 cases were relatively satisfied, 9 cases were satisfied, and the patient satisfaction score was (85.81 ± 5.49) points, the difference between the two groups was statistically significant ( χ2 value was 23.312 2, t value was - 7.205 7, P < 0.01).There were no difference of arterial blood gas results between the two groups ( P>0.05) .Conclusion:Early active exercises can shorten the weaning time of mechanical ventilation patients with MG, improve patients comfort and satisfaction with nursing services during the weaning time, and have no effect on patients' respiratory function and acid-base balance.

ABSTRACT

Limb shaking transient ischemic attack (LS-TIA) can be recognized by paroxysmal cerebral hemisphere hemodynamic damage performance that lasts for several seconds or minutes, often occurring after exercise or standing and may be accompanied by numbness of the local limb. Clinically LS-TIA is ignored by doctors and misdiagnosed as epilepsy and the disease has a high risk of stroke and poor prognosis. The prompt recognition of the limb-shaking transcient ischemic attack is crucial to implement options aiming at preventing future stroke. Patients with LS-TIA can benefit from therapies aiming at improving cerebral perfusion.However, the benefit of revascularization and drug conservative treatment remains to be determined and specific diagnosis and treatment should be formulated according to the specific clinical characteristics of patients.

ABSTRACT

OBJECTIVE@#To study the effects of Transient receptor potential cation channel, subfamily V, member 1 (TRPV1) combined with lidocaine on status and apoptosis of U87-MG glioma cell line, and explore whether local anesthetic produces neurotoxicity by TRPV1.@*METHODS@#U87-MG cells were divided into control group, gene silencing group, empty vector group and TRPV gene up-regulation group. For cells in each group, flow cytometry was employed to detect the intracellular calcium ion concentration and mitochondrial membrane potential at different time point from cellular perspective. Cell apoptosis of U87-MG was assayed by flow cytometry and MTT from a holistic perspective.@*RESULTS@#Calcium ion concentration increased along with time. The concentration in TRPV1 gene up-regulation group was significantly higher than those in other groups at each time point (P < 0.05). After adding lidocaine, mitochondrial membrane potential in U87-MG significantly increased (P < 0.05). This increasing trend in TRPV1 gene up-regulation group was more significant than other groups (P < 0.05), while in TRPV1 gene silencing group, the trend significantly decreased (P < 0.05). Flow cytometry result and MTT result both showed that cell apoptosis in each group significantly increased after lidocaine was added (P < 0.05). This increasing trend in TRPV1 gene up-regulation group was more significant than other groups (P < 0.05), while in TRPV1 gene silencing group, the trend significantly decreased (P < 0.05). Moreover, apoptosis was more severe along with the increasing concentration of lidocaine (P < 0.05).@*CONCLUSIONS@#In this study, it was proved that lidocaine could dose-dependently induce the increase of intracellular calcium ion concentration, mitochondrial membrane potential and apoptosis in U87-MG glioma cell line. The up-regulation of TRPV1 enhanced cytotoxicity of lidocaine, which revealed the correlations between them. Lidocaine might have increased intracellular calcium ion concentration by activating TRPV1 gene and induced apoptosis of U87-GM glioma cell line by up-regulating mitochondrial membrane potential.
